First complete this survey for yourself. Then distribute it to your fellow team members to complete. Review the feedback, conduct clarifying discussions and set some growth goals backed up by a plan of action. Share your goals and action plan with your fellow team members and ask them to give you "on-course" affirmative feedback and "off-course" corrective feedback to help your grow and go to greater levels of excellence and contribution to the work of the team.
